In the state of Wisconsin, it is illegal to tattoo a minor, even with parental consent.

Wis. Stat. §948.70

Tattooing of a Minor. Prohibits anyone other than a physician in the course of his or her professional practice from tattooing or offering to tattoo a child (under age 18).
